I understand that yoga includes physical movements as well as an opportunity for relaxation, stress re-education and relief of muscular tension. As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. If I experience any pain or discomfort, I will listen to my body, adjust the posture and ask for support from my teacher. I will continue to breathe smoothly. Yoga is not a substitute for medical attention, examination, diagnosis or treatment. Yoga is not recommended and is not safe under certain medical conditions. I affirm that I alone am responsible to decide whether to practice yoga. By signing below, I hereby agree to irrevocably release and waive any claims that I have now or hereafter may have against Jennifer McKinney (Mint Condition Personal Training). 1. Training sessions will range from 45-60 minutes, depending on the type of workout planned. 2. The expiration policy requires a completion of all yoga sessions within 60 days from the date of the package purchase. Sessions are void after this time period. Please discuss family emergencies and travel conflicts with Jennifer to arrange a hold on current training sessions. 3. Make up sessions will be on an availability basis, based on the current training schedule. 4. As a curtesy you will provide a 2 weeks notice of discontinued services.
